The contact owns an Evenflo Car Seat, Model Number: 368124708, Model Type: Revolve 360 Slim, Manufactured Date: July 11, 2024. The car seat was being used in a 2023 Toyota Highlander. The contact received notification of NHTSA Campaign Number: 25C010000 (Child Seat). The contact stated that the head rest foam was exposed on the left and right sides of the headrest and that the child was picking out the foam as described in the recall. The manufacturer was contacted and provided pictures of the failure. The contact was advised by a representative from the manufacturer that a repair kit would be sent in the mail to repair the car seat. The contact received the repair kit and found an 8-inch-long by 3/12-inch-wide strip of tape. The contact stated that the repair kit was inadequate to complete the repair on the head rest. The contact had not attempted to call the manufacturer back about the repair kit.

Dear NHTSA, I am writing to file a complaint regarding Recall No. 25C010000 involving the Evenflo Gold Revolve360 Slim child seats. The recall identifies a choking hazard from foam in the headrest, which could be life-threatening for children. The current remedy offered by Evenflo is a tape seal kit with installation instructions. I believe this is not an adequate or safe solution for the following reasons: 1.Children’s safety: Foam pieces can still detach or be accessed by the child, even with tape. A taped fix is not equivalent to a redesign or properly secured headrest. 2.Temporary remedy: Tape is not a permanent repair for a product designed to last years. It will degrade over time, especially under heat, cleaning, or regular use. 3.Consumer expectation: Parents should not be expected to modify or repair a safety-critical product themselves. For these reasons, I respectfully request that NHTSA require Evenflo to provide a refund or full replacement car seat to affected owners. Nothing less than a secure, manufacturer-tested headrest is acceptable for child safety. Thank you for your attention to this urgent matter. I trust NHTSA will ensure that families receive a safe and adequate remedy.

The latch holding the seatbelt failed as well as the notification that the latch was unlocked. We noticed that our seat would not revolved, we removed the seat from the base and the latch locking the base to the seatbelt was unlocked. Additionally, the latch, when unlocked, was green which indicated that the seat was locked, and it was absolutely not. The latch would remain green when flipped up completely in the unlocked position. Evenflo was contacted via video and agreed that the latch was malfunctioning.
