    The harness loosens after being tightens. I have noticed on several occasions after a drive significant slack. We are discontinuing use after seeing online that this is a common problem.

    FOREGROUND: The pivot point lock of the base failed. Images attached. If a hard stop occurred, the shell and secured child within would be a flying projectile. The failure is only seen if the shell is lifted off the base, which only occurs during reinstallations. The failure can also be completely missed if the pivot point lock stays lodged in the shell during reinstallation rather than being seated within the base assembly. The car seat is being sent back to the manufacturer as soon as I receive the new one they promised. I am willing to hold onto it for inspection if requested. INCIDENT: Car seat was being moved from one vehicle to another. During reinstallation, the locking pivot point stayed lodged in the shell and "locked". For the consumer, nothing looks missing, because the lodged pivot point can still fit back into place on the base, while still lodged in the car seat. When double checking the belt tightness on the base, the lodged pivot point fell out of the shell. It was then noticed that the keys on the pivot point that lock and hold the shell to the base are completely fatigued. If it had not fallen out of the shell, I would not have noticed the failure. If we had come to a complete stop with it installed broken, the shell would have flown with child secured inside. Death or injury could have occurred for the child, and injury for those that could potential be hit by the shell in a side impact crash.

    I went to unbuckle my [XXX] old today and she had the inside parts to the sensor safe car seat chest clip in her hands. The internal components of the chest clip including the button battery were in my child’s hand and she could have easily swallowed it had I not found it when I did. I still have all parts that are available to be inspected if needed. This presented a major choking hazard and button battery consumption risk. The car seat has not been inspected by anyone but photos were sent to the manufacturer who is sending a replacement of the same seat.     I would like to report a potential safety concern involving the Evenflo Revolve360 Slim 2-in-1 Rotational Car Seat. My child is [XXX] old and is able to independently press the harness release/loosening button located between her legs while seated in the car seat during normal vehicle operation. Once she presses the button, she is able to lean forward, loosen the harness straps significantly, and climb out of the seat on her own while the vehicle is in motion. This has happened repeatedly during everyday use and not during a crash. My concern is that the harness release mechanism is too accessible to a young child and allows the child to defeat the restraint system while riding in the vehicle. I am concerned that if this occurred during a collision or sudden stop, my child could be seriously injured or ejected from the seat due to the loosened harness. The seat has been used according to manufacturer instructions to the best of my knowledge. I believe this may represent a design or safety issue that should be reviewed further. I am submitting this complaint because I believe other caregivers may experience the same issue, and I am concerned about the risk of injury associated with a child being able to loosen the harness independently during normal travel.     The harness webbing/strap on the Evenflo Revolve360 Slim tore without any known incident or accident causing the damage. The defect was discovered on April 15, 2026 when the seat was temporarily removed from the vehicle. Upon inspection, the harness strap was found to be nearly completely torn through and would likely not have restrained the child occupant in the event of a crash. The seat had been in regular use and had not been involved in any collision. No unusual event or misuse is known to have caused the failure. It is suspected that the rotating mechanism of the seat placed repeated stress on the harness webbing over normal use, causing it to deteriorate and tear. The seat has not yet been inspected by the manufacturer, law enforcement, or insurance representatives, but is available for inspection upon request. The seat has been taken out of service and is not being used.

    We have experienced a mechanical issue with the harness system. Specifically, the straps become extremely difficult to tighten or loosen over time. To adjust them, I have to pull with the full force of a 200lb adult, to the point where it literally shakes the car. Despite following all manufacturer instructions—including cleaning the adjuster area and checking for twisted straps—the problem persists. Evenflo recently closed our support case (#00609796) and sent a replacement seat, which suggests an acknowledgement of the problem. Before I return the impacted seat (as Evenflo Requested), I wanted to reach out here promptly. I am particularly concerned because this appears to be a systemic issue.     The contact owns a 2025 Toyota Grand Highlander with an Evenflo Revolve 360 Slim car seat, Model Number: 36812470A, Manufactured Date: December 12, 2022, installed in the vehicle. The contact received notification of NHTSA Campaign Number: 25C010000 (Child Seat). The contact stated that while driving at an undisclosed speed with the child secured in the car seat, the contact became aware that there was an object in the child’s mouth. The contact pulled over to the side of the road and removed a foam piece from the child's mouth. The contact inspected the car seat and became aware that the foam was from the headrest. Medical attention was not needed. The manufacturer was not notified of the failure, but the contact stated that the manufacturer mailed tape to cover the exposed areas of the car seat.
