Rollx Vans (Rollx) is recalling certain GMC Yukon, Sierra, Chevrolet Express 2500, Ford Transit, E-250, Toyota Sienna, Sienna Hybrid, Honda Odyssey, Chrysler Town and Country, Voyager, Pacifica, Dodge Grand Caravan, and Ram Promaster vehicles equipped with QRT-Deluxe and QRT-Max wheelchair restraints. Please refer to Rollx's recall report for specific model year information. The retractors may not lock, preventing the wheelchair from being properly secured.

The recall was influenced by the manufacturer with about 86 units affected.

Description:: Rollx Vans (Rollx) is recalling certain GMC Yukon, Sierra, Chevrolet Express 2500, Ford Transit, E-250, Toyota Sienna, Sienna Hybrid, Honda Odyssey, Chrysler Town and Country, Voyager, Pacifica, Dodge Grand Caravan, and Ram Promaster vehicles equipped with QRT-Deluxe and QRT-Max wheelchair restraints. Please refer to Rollx's recall report for specific model year information. The retractors may not lock, preventing the wheelchair from being properly secured.
Consequeces:: An unsecured wheelchair can move during transit, increasing the risk of injury.
Corrective action:: Rollx will work with Q'Straint to inspect and replace the retractors as necessary, free of charge. Owner notification letters were mailed February 5, 2026. Owners may contact Rollx's customer service at 1-800-956-6668.

2011 FORD E-250 Complaints
THE PCM AT 34,000 MILES CAUSED TWO FUEL PUMPS TO BURN OUT IN 4 DAYS. A "WRENCH" WARNING SYMBOL APPEARED ON THE DASH AND THE VANS ENGINE DIES WHILE GOING AT HIGH SPEED (WE PULL A PRESSURE WASHING TRAILER HOLDING WITH HAZARDOUS CHEMICALS & WATER). IF IT WAS RAINING OR EVEN HEAVY TRAFFIC THIS COULD CAUSE A DANGEROUS LIFE THREATENING CRASH. ON WET ROADS THESE HEAVY COMMERCIAL VEHICLES CAN EASILY HYDRO-PLAIN WHEN THIS OCCURS PUTTING THE DRIVER & OTHERS IN DANGER WHO MAY BE IN THE PATH. WE ARE NOW WITHOUT OUR SERVICE VAN FOR 2 WEEKS & FORD ESTIMATED IN THE EXCESS OF $2600 IN REPAIRS ON A VEHICLE WITH JUST 34,000 MILES ON THE VAN.WE JUST REPLACED THE TRANSMISSION 3 MONTHS AGO. MY STEP FATHER IS 84 YEARS OLD WHO OWNS THIS VEHICLE AND HE IS A RETIRED NAVY CAPTAIN OF OVER 25 YEARS. HE AND I ARE BOTH CONCERNED THAT THIS VEHICLE IS TOO DANGEROUS TO DRIVE AT HIGH SPEEDS (60-70MPH) WITH THE LOADS WE CARRY IN OUR EVERY DAY SERVICE SCHEDULE. FORD SEEMS TO OFFER NO ASSISTANCE FINANCIALLY AND NO COMFORT IN RESOLVING THE SAFETY ISSUES RESULTING IN THE FAILURE OF THE PCM (POWER TRAIN CONTROL MODULE) WHICH IS OBVIOUSLY VERY DANGEROUS WHEN LARGE COMMERCIAL VEHICLES HAVE THE ENGINE FAILING OR DYING AT HIGH SPEEDS. AFTER READING ABOUT THE THROTTLE BODY PROBLEMS THAT FORD EXPERIENCING WITH OVER 1,600,000 VEHICLES BEING RECALLED, IT SEEMS THAT THE SAME WARNING SYMBOL OF THE TWO SIDED WRENCH APPEARS ON THE DASH AND THE SAME ENGINE FAILING OCCURS AT HIGH SPEEDS. THE SERVICE MECHANIC AT BIG STAR FORD IN MANVEL TX. TOLD ME HE REPLACED THE FIRST FUEL PUMP, DROVE IT ON THE HWY AND HAD TO TOW IT BACK TO THE DEALER BECAUSE IT DIED WHILE TEST DRIVING THE VAN AND BURNED OUT THE NEW FUEL PUMP HE JUST INSTALLED, THEN HE SAID HE PUT A NEW FUEL PUMP IN AGAIN AND USED THAT TO DIAGNOSE THAT THE PCM WAS THE PROBLEM WHILE RUNNING IT ON IDLE FOR AN HOUR. WE ARE NOW CONCERNED THIS NEW PUMP IS DAMAGED.;
2015 CHRYSLER TOWN AND COUNTRY Complaints
Transmission completely failed and had to be risky rebuilt. I was just driving down the highway and the transmission went into limp mode and I came to a fast slow down as it dropped the gearing. Also have noticed a huge issue with electronics putting me into a limp mode with wipers on headlights wont shut off, lose dash lights and speedometer with no blinkers or hazard lights. This left me in a very unsafe place as I had no way to tell other drivers where I was going or see how fast im going. Could have caused a huge accident ??. Please look into this as there are many issues of this happening.;
2015 HONDA ODYSSEY Complaints
2015 Honda Odyssey Touring Elite, VIN [XXX]. Vehicle was involved in a frontal collision on XXX. The driver's frontal airbag did not deploy. During the impact, I was thrown forward and struck the steering wheel. Additionally, a side/sliding door opened during the collision. The door was closed before the collision. 2017 TOYOTA SIENNA Complaints
The fuel filler door assembly on this 2017 Toyota Sienna failed structurally due to a defective hinge mechanism. This defect directly compromises a critical vehicle safety system. The Sienna is engineered with a physical safety interlock cable that links the fuel door to the driver-side sliding passenger door. This system is designed to prevent the heavy power-sliding door from operating while the fuel cap is off, protecting occupants from injury and preventing the door from violently striking a gas station fuel pump nozzle during refueling. Because the fuel door assembly failed, the safety interlock mechanism was compromised, presenting an immediate risk of fuel pump damage, fuel spillage, or passenger injury from the sliding door. Toyota corporate previously issued a Customer Satisfaction Program (CSP) to address this specific structural defect. However, my local franchised Toyota dealership refused to preform comprehensive repair, claiming they "only cover the pin". This dealer non-compliance forced me to seek immediate out-of-pocket repairs at an adjoining facility to restore the vehicle's structural safety interlock system. Toyota Corporate is refusing to remediate this safety-related failure. I have made several attempts to resolve this directly with Toyota Corporate (6 or more), without success. This included several phone calls, and a letter written to the Toyota Brand Engagement Center, which went unanswered.;
2020 DODGE GRAND CARAVAN Complaints
The automatic transmission failed while driving, causing a loss of propulsion and leaving the vehicle stranded. This created a safety hazard because the vehicle could no longer accelerate or move out of traffic. This is the fourth Chrysler/Dodge vehicle I have owned that has experienced a substantially similar transmission failure.;
