    The same issue that is described in the recall information for the Evenflo Revolve360 Slim car seats. We were finding bits of Styrofoam on our child unaware of where it was coming from. Upon observation and inspection, while seated in the Extend model car seat child is reaching behind the cushion, picking at, and dislodging the foam from the headrest. Carseat manufacturer has been made away of the situation.

    I am reporting a potential safety defect involving an Evenflo Revolve360 child restraint. Manufacturer: Evenflo Product: Revolve360 Model: CS360212464 Manufactured: September 26, 2025 Purchased: XXX from Target The seat was used rear-facing by my [XXX] daughter, who weighs approximately 27 pounds. After only about four months of normal use, I discovered significant localized fraying/tearing of the harness webbing. I have photographs documenting the damage. The damage appears concentrated in a specific area of the harness webbing near where it passes through/contacts components of the seat. I do not see corresponding damage to the surrounding fabric, foam, plastic, metal structure, or other visible areas that would readily indicate an environmental cause. I contacted Evenflo and provided photographs. Evenflo stated that the damage “appears to be related to external environmental factors” and that the components contacting the strap are smooth and rounded. However, Evenflo did not identify what specific environmental factor caused the damage or what physical evidence supports that conclusion. Evenflo also advised that the seat could be used in booster mode if my child met the requirements. This is not an applicable solution because my daughter is only [XXX] old, [XXX] pounds, and still rear-facing. My concern is product safety: a critical harness component experienced substantial deterioration after approximately four months of use. I am requesting that NHTSA record this as a child-restraint safety complaint and evaluate whether this harness deterioration could represent a potential safety defect or pattern involving the Revolve360. I can provide photographs, proof of purchase, product labels, Evenflo correspondence, and the physical seat if needed.     Swivel motion has started to jam when half locked, base will show half red/half green and refuse to lock. Unlock handle becomes hard to lift to swivel the car seat back to attempt to lock it again. I’ve contacted Evenflo, but waiting to hear back.

    This car seat is missing any labels on the seat that have booster information. It is an all-in-one car seat and does have a booster mode. The manual does state the height, weight and age limit, but this is not noted anywhere on the stickers on the seat. This is a problem when the manual is not available.

    Subject / Issue: Child car seat caused vehicle seat belt to become permanently jammed Complaint: I am reporting a safety-related issue involving an Evenflo Revolve 360 child car seat. During normal use, the vehicle seat belt became slightly twisted inside the car seat base, and the seat stopped rotating properly. We were then unable to remove the child car seat from the base following the standard removal instructions. While attempting to safely remove the seat, the vehicle seat belt became completely jammed inside the base. The seat belt is now fully immobilized and cannot move in either direction. As a result, the child car seat is permanently stuck in the vehicle, and the seat belt system in the left rear seat is unusable. We had the vehicle inspected at a Toyota service center, and they confirmed that the only way to remove the child seat is to cut and replace the left rear seat belt. The estimated repair cost is approximately $680. This situation has rendered a vehicle safety restraint system inoperable and requires cutting the seat belt to resolve the issue. I am concerned about the safety design and potential risk if this occurs during normal consumer use. Photos of the issue are available and can be provided if needed.

    I discovered significant fraying and tearing of the harness webbing on an Evenflo Revolve360 Extend Revere child restraint seat during routine inspection. The damage is located on the harness strap near the seat pan opening where the harness passes through the restraint system. The damaged seat and harness are available for inspection. The seat was purchased new in April 2023 and has been used only as intended. It remained continuously installed in the vehicle, was never involved in a crash, was never stored, and was used exclusively in a rear-facing configuration. The child using the seat was approximately 22 pounds and 33 inches tall when the damage was discovered. The harness is a critical safety component designed to restrain a child during a collision. The fraying and tearing involved multiple strands of the webbing and appeared severe enough that the harness could potentially be weakened and unable to perform as intended in a crash. Because of the condition of the harness, the child's safety may have been compromised if the damage had gone unnoticed. The manufacturer, Evenflo, reviewed photographs of the damaged harness. Evenflo stated that the damage was caused by unspecified "environmental factors" and not by the design or manufacture of the product. However, no specific environmental factor was identified. Evenflo also advised that the seat should no longer be used with the harness because of the damage. My concern is that a critical child restraint component experienced substantial deterioration after approximately two years of normal use despite the seat being used according to instructions, remaining installed in the vehicle, and not being subjected to any known misuse, crash, or unusual conditions. Photographs and manufacturer correspondence are available and being submitted with this complaint.

    On the car seat there are two indicators to tell you if the seat is locked in place because it is a rotating car seat. Both indicators never fully go in the “green” meaning the seat is fully locked into place. The car seat is less than two years old and both indicators are malfunctioning, they aren’t going fully in the green. I reached out to the company with pictures and videos and they have done nothing to help. They said from the video it looks like the car seat is functioning properly and they will not fix or replace it.

    I was able to follow the instructions to secure the base with absolutely no movement even with force. When placing the seat into the base, despite locking it into rear facing with audible click and lock guide green, the seat “wobbles” excessively, more than 1 inch teetering sideways and 1/2 inch back and forth swiveling despite the locked status into the base. I was told the actual seat part is SUPPOSED to move/wiggle within the base and there is no defined limits of how much would be unsafe or not within the design normal. I have video evidence. I was within the return window and returned the product.

    The contact owns an Evenflo Safety Seat, Model Type: Revolve Extend 360 Rotational All-in-One Convertible Car Seat, Model Number: 25701756, manufactured in October 2022. The safety seat was being utilized in a 2025 Buick Enclave. The contact stated that while driving at approximately 35-45 MPH, the vehicle was T-boned by another vehicle on the passenger side. The air bags did not deploy. The safety seat was located on the rear driver's side, and another safety seat was on the rear passenger's side; however, the rear passenger's side safety seat showed no visible signs of damage. There were no passengers in the safety seats. The contact stated that the rear driver's side safety seat was in rear-facing position, but after the crash, the safety seat had rotated to forward-facing and had separated from its detachable base, landing on its back. The contact sustained mild whiplash and received medical attention. A police report was filed. The vehicle was towed to a tow yard but not yet deemed totaled. The contact related the failure to NHTSA Action Number: AQ24001 (Child Seat). The safety seat manufacturer was not made aware of the failure.
