FUEL/PROPULSION SYSTEM

7/8/2014 - NEW LENOX, IL
I STOPPED FOR GAS AND WAS UNABLE TO OPEN THE FUEL FILLER DOOR. BECAUSE OF THE FUEL FILLER DOOR FAILURE, I WAS NOT ABLE TO OPEN THE DRIVER SIDE PASSENGER DOOR. I DROVE THE VEHICLE HOME AND UPON FURTHER INVESTIGATION, I DISCOVERED THE TOP FUEL FILLER DOOR HINGE WAS BROKEN FREE. THE BROKEN HINGE ALLOWED THE PASSENGER DOOR ACTIVATION SWITCH LOCATED BEHIND THE FUEL FILLER DOOR TO POP OUT WHICH DE-ACTIVATED THE PASSENGER DOOR OPERATION. IN THE EVENT OF AN ACCIDENT, THE DRIVER SIDE PASSENGER DOOR WOULD NOT BE ABLE TO BE OPENED.

6/16/2014 - MERCER ISLAND, WA
2011 TOYOTA SIENNA. WHILE DRIVING ABOUT 40 MPH, THE POWER STEERING FAILED AS THE STEERING TRACTION CONTROL AND THE VSC WARNING LIGHTS ILLUMINATED. STEERING WHEEL IS VERY HEAVY AND HARD TO TURN. WHEN PARKING OR TURNING AT A ROAD CROSS THE STEERING WHEEL SEIZED.

DRIVING APPROX. 45 MILES PER HOUR IN THE CITY AND SUDDENLY THE PASSENGER SIDE REAR BACK WINDOW (VENT WIDOW) SHATTERED. FIRST A VERY LOUD NOISE AND THEN THE GLASS SHATTERED AND WENT EVERYWHERE. THERE WAS NO VISIBLE DAMAGE NOR DID THERE SEEM TO BE ANY EXTERNAL DEBRIS THAT COULD HAVE HIT THE WINDOW. THE ROAD WAS RATHER SMOOTH. NOT SURE IF THIS IS A COMMON PROBLEM FOR WINDOWS OR THE 2011 TOYOTA SIENNA.

5/12/2014 - MONSEY, NY
THE CONTACT OWNS A 2011 TOYOTA SIENNA. THE CONTACT STATED THAT WHILE DRIVING 30 MPH, THE POWER STEERING FAILED AS THE STEERING TRACTION CONTROL AND THE VSC WARNING LIGHTS ILLUMINATED. THE VEHICLE WAS TAKEN TO AN INDEPENDENT MECHANIC WHERE THE TECHNICIAN DIAGNOSED THAT THE STEERING COLUMN NEEDED TO BE REPLACED. THE MANUFACTURE WAS NOT MADE AWARE OF THE FAILURE. THE FAILURE MILEAGE WAS 58,545 AND THE CURRENT MILEAGE WAS UNKNOWN.

THE CONTACT OWNS A 2011 TOYOTA SIENNA. THE CONTACT STATED THAT WHILE REVERSING FROM A PARKING SPACE, THE STEERING SEIZED WITHOUT WARNING. THE CONTACT WAS NOT ABLE TO TURN IN EITHER DIRECTION AND HAD TO RESTART THE VEHICLE TO REGAIN STEERING CAPABILITIES. THE CONTACT ALSO STATED THAT ON A SEPARATE OCCASION, THE KEY COULD NOT BE TURNED IN THE IGNITION. THE CONTACT HAD TO CALL ROADSIDE ASSISTANCE TO GET THE VEHICLE STARTED. THE VEHICLE WAS TAKEN TO THE DEALER HOWEVER, THEY WERE UNABLE DIAGNOSE THE FAILURE. THE VIN WAS NOT AVAILABLE. THE FAILURE MILEAGE WAS 24,000.

4/19/2014 - STUART, FL
THE AIR CONDITIONER HOUSING IS LEAKING WATER ONTO SAFETY-CRITICAL PARTS. THIS HOUSING IS MADE OF PLASTIC PARTS THAT ARE GLUED TOGETHER. WATER IS CONDENSED DURING THE COOLING CYCLE, AND IT THEN COLLECTS AT THE BOTTOM AND EXITS THE VEHICLE THROUGH A RUBBER HOSE. THE GLUE HOLDING THE HOUSING TOGETHER HAS FAILED. WATER IS LEAKING FROM THE HOUSING AND RUNNING ONTO THE COMPUTER MODULE THAT DEPLOYS THE AIRBAGS. I HAVE BEEN ADVISED BY THE DEALER THAT IT IS NOT SAFE TO DRIVE THE VEHICLE WITH A WET MODULE BECAUSE THE AIRBAGS CAN DEPLOY WHILE MOVING, OR THAT THEY MIGHT NOT DEPLOY IN THE EVENT OF A COLLISION. AN IDENTICAL PROBLEM AFFECTS TOYOTA MODELS CAMRY, AVALON, AND VENZA. THIS RECALL CAN BE FOUND AT: WWW.TOYOTANEWS.PRESSROOM.TOYOTA.COM THE RECALL INFO IS: RELEASES/TOYOTA/RECALL+CAMRY+AVALON+VENZAOCT17 THIS WILL TURN INTO A SERIOUS SAFETY ISSUE AS THESE VEHICLES GET OLDER WATER CAN LEAK ONTO THE DEPLOYMENT MODULE WITHOUT BEING SEEN. IN MY PARTICULAR CASE, THE WATER HAD APPARENTLY BEEN LEAKING FOR SOME TIME - PERHAPS A YEAR OR TWO - AND WAS DETECTED PURELY BY CHANCE. THERE WAS SIGNIFICANT RUST ON THE FLOORBOARD OF THE VEHICLE. I HAVE DISCUSSED THE ISSUE WITH TOYOTA, AND THEY REFUSE TO DO ANYTHING ABOUT IT. THE CIRCUMSTANCES OF THE RECALL ON CAMRY, AVALON, AND VENZA APPEAR TO BE IDENTICAL TO THE CIRCUMSTANCES I HAVE EXPERIENCED, YET THERE IS NO RECALL ON THE SIENNA. THIS APPEARS TO BE AN UNSAFE CONDITION. I WOULD APPRECIATE IT IF YOU WILL TAKE A LOOK.
